Description
Deliver vocational counseling and case management to help individuals with disabilities or employment barriers achieve independence and competitive employment. Assess vocational needs, coordinate evaluations and training, develop individualized employment plans, arrange accommodations, and collaborate with employers, providers, and families to support placement and retention.
- • Monitor and document clients' progress toward vocational goals and outcomes.
- • Meet with clients to explore strengths, interests, and barriers, and develop individualized plans for employment.
- • Prepare and maintain case records, eligibility documentation, service plans, and authorizations.
- • Arrange vocational, psychological, and functional evaluations to inform eligibility and planning.
- • Analyze interview, medical, educational, and test data to assess vocational readiness and service needs.
- • Develop employment plans aligned with clients' aptitudes, education, abilities, and labor market demands.
- • Provide or coordinate job readiness services, including resume development, interviewing, and workplace soft skills.
- • Identify employment barriers (e.g., transportation, accessibility, schedules) and create strategies to overcome them.
- • Coordinate workplace accommodations and assistive technology; arrange on-site job coaching as needed.
- • Collaborate with physicians, psychologists, therapists, educators, and benefits counselors to align services.
- • Provide or refer for benefits counseling regarding SSI/SSDI work incentives and the impact of earnings.
- • Conduct labor market research and evaluate job matches for suitability and career growth.
- • Manage case budgets and authorize services; track expenditures and ensure fiscal compliance.
- • Ensure adherence to ethical, legal, and confidentiality standards; prepare required reports and metrics.
- • Maintain contact during training, internships, and job placements to resolve issues and support retention.
- • Build and sustain relationships with employers and community partners to support job development and placement.
