Description
Counsel individuals to maximize independence and employability while addressing personal, social, and vocational challenges stemming from disability, illness, injury, aging, or life stressors. Assess needs and design, implement, and coordinate individualized rehabilitation plans, providing counseling, training, job placement, and services across care facilities and community settings.
  • • Monitor and record clients' progress to ensure goals are met.
  • • Confer with clients to set goals and develop rehabilitation plans and service access.
  • • Prepare and maintain case records, including eligibility, services, contact notes, and correspondence.
  • • Arrange needed evaluations (physical, psychological, academic, vocational) to inform assessments and plans.
  • • Analyze interviews, records, consultations, and evaluations to assess abilities, needs, and eligibility for services.
  • • Develop individualized rehabilitation plans aligned with aptitudes, education, abilities, and career goals.
  • • Support clients during training and placements, resolving issues and evaluating fit.
  • • Build and maintain relationships with community referral sources (e.g., schools, agencies, community groups).
  • • Identify employment barriers (e.g., access, schedules, transportation) and develop strategies to overcome them.
  • • Arrange on-site job coaching and assistive technology to support work or school adaptation.
  • • Collaborate with physicians, psychologists, occupational therapists, and other professionals to develop and implement programs.
  • • Select and apply diagnostic procedures to determine clients' needs.
  • • Participate in job development and placement, engage employers, place clients, and evaluate outcomes.
  • • Collaborate with clients' families to support behavioral, residential, social, and employment goals.
  • • Coordinate with community agencies to expand access to services and programs for persons with disabilities.
  • • Manage case service budgets and authorize expenditures.
  • • Coordinate with and, when assigned, mentor or supervise support staff.
