Description
Provide vocational counseling and case management to individuals with disabilities to achieve competitive integrated employment and greater independence. Assess needs, develop Individualized Plans for Employment, coordinate evaluations and training, secure accommodations, and place and retain clients through employer outreach and ongoing support.
  • • Conduct intakes and vocational assessments using interviews, records, and standardized tools.
  • • Develop Individualized Plans for Employment (IPEs) aligned with clients’ abilities, interests, and labor market demands.
  • • Provide counseling and guidance on career exploration, training options, and disability-related concerns.
  • • Arrange medical, psychological, functional, and vocational evaluations to inform planning.
  • • Coordinate skills training, education, certifications, and work-based learning to build employability.
  • • Identify and address barriers to employment, including transportation, childcare, and accessibility.
  • • Advise on reasonable accommodations and assistive technology; arrange job coaching and worksite modifications.
  • • Conduct labor market research and job matching to target suitable opportunities.
  • • Develop employer relationships; market client strengths; facilitate interviews and job development.
  • • Coordinate return-to-work plans with employers, physicians, and insurers as appropriate.
  • • Provide benefits counseling on the impact of work and available work incentives.
  • • Monitor and document progress during training and employment; troubleshoot issues to promote retention.
  • • Maintain accurate case files, eligibility documentation, and service authorizations in compliance with state and federal VR regulations.
  • • Collaborate with families, schools, and community providers to support employment goals.
  • • Prepare and manage service budgets; authorize and track payments for approved services.
  • • Collect outcome data; evaluate placement quality; report on performance measures and case closures.
  • • Arrange transportation, interpreters, or other supports to improve access to services.
  • • Participate in outreach and referral development with community agencies and employers.
