Description
Manage vocational rehabilitation cases to help individuals with disabilities, injuries, or other barriers prepare for, obtain, and retain employment. Assess strengths and needs, develop individualized plans, coordinate evaluations, training, and supports, engage employers, monitor outcomes, and maintain compliant documentation throughout the service continuum.
  • • Conduct intake, determine eligibility, and establish vocational case objectives.
  • • Develop Individualized Plans for Employment (IPEs) with services, timelines, and measurable outcomes.
  • • Prepare, maintain, and audit case records, authorizations, and required reports.
  • • Arrange and coordinate medical, psychological, vocational, and functional evaluations.
  • • Analyze assessments, records, and provider input to identify abilities, limitations, and service needs.
  • • Coordinate training, education, and work-readiness services aligned with client goals and labor market demand.
  • • Provide job development support, contact employers, and facilitate interviews and work tryouts.
  • • Monitor client progress during training, internships, and placements; resolve issues and adjust plans.
  • • Identify employment barriers, such as transportation, childcare, or accessibility, and broker supports to address them.
  • • Arrange job coaching, workplace accommodations, or assistive technology to support job retention.
  • • Collaborate with healthcare providers, therapists, and service vendors to implement rehabilitation or return-to-work plans.
  • • Educate clients on work incentives, benefits, and rights under applicable employment and disability laws.
  • • Develop and maintain relationships with community partners, schools, and referral sources.
  • • Manage case budgets; authorize services and track expenditures in line with funding requirements.
  • • Coordinate logistics such as scheduling, transportation, and interpreter or assistive services.
  • • Collect and analyze outcome data to evaluate placement quality, retention, and wage progression.
  • • Prepare employer proposals and reasonable accommodation plans; mediate workplace concerns as needed.
  • • Ensure compliance with agency policies, funder regulations, and privacy requirements.
