Description
Guide clients facing disability, illness, or life stressors to access care, benefits, and employment supports. Assess needs, develop action plans, coordinate appointments, transitions, and services across providers and community programs, remove barriers, and track progress toward independence and work readiness.
  • • Monitor and document client progress toward service, health, and employment goals.
  • • Meet with clients to clarify goals, options, and next steps; co-create navigation plans.
  • • Maintain accurate case files, eligibility documents, service logs, and contact notes.
  • • Coordinate referrals for medical, behavioral, vocational, and benefits evaluations.
  • • Assess needs and eligibility using interviews, records, and consultations with providers.
  • • Develop individualized service and navigation plans aligned with abilities and aspirations.
  • • Stay in close contact during appointments, trainings, and placements to resolve issues.
  • • Build and sustain relationships with community partners and referral sources.
  • • Identify barriers to access or employment and coach clients on strategies to overcome them.
  • • Arrange accommodations, assistive technology, transportation, or job coaching as needed.
  • • Collaborate with healthcare, behavioral health, and rehabilitation professionals to implement plans.
  • • Create or refine intake, screening, and triage workflows to prioritize client needs.
  • • Support job development by connecting clients to training, employers, and workforce programs; follow up on placement outcomes.
  • • Engage clients’ families or supports to advance behavioral, residential, social, or employment goals.
  • • Coordinate with community agencies to expand resource networks and streamline service pathways.
  • • Manage service authorizations, track benefits and funding sources, and approve covered services within scope.
  • • Provide guidance and training to volunteers, peer navigators, or interns; share best practices with the team.
