Description
Provide specialized rehabilitation services to improve independence, community participation, and employability for individuals with disabilities or health-related barriers. Conduct assessments, develop and implement individualized plans, coordinate interdisciplinary services and supports, and facilitate training, accommodations, and job placement.
- • Monitor and document client progress toward individualized rehabilitation goals.
- • Meet with clients to clarify goals, options, and service preferences to shape plans.
- • Maintain accurate case records, eligibility data, service authorizations, and contact notes.
- • Coordinate and schedule medical, psychological, functional, academic, and vocational evaluations.
- • Synthesize interview data, records, and provider input to determine abilities, needs, and eligibility.
- • Design and update individualized rehabilitation and return-to-work plans aligned with strengths and goals.
- • Provide support during training and placements; troubleshoot issues and assess fit.
- • Build and sustain referral networks with schools, workforce agencies, and community groups.
- • Identify employment and participation barriers and coach clients on strategies to overcome them.
- • Arrange job coaching, workplace accommodations, and assistive technology to support adaptation.
- • Collaborate with physicians, therapists, and other specialists to coordinate integrated services.
- • Develop and refine assessment protocols and outcome measures for client needs.
- • Lead job development and employer outreach; place clients and evaluate outcomes.
- • Engage families and caregivers to reinforce behavioral, residential, social, or employment goals.
- • Partner with community agencies to expand accessible programs and resources.
- • Manage service budgets, authorize expenditures, and track utilization.
- • Train and mentor rehabilitation staff or interns and contribute to quality improvement.
