Description
Counsel individuals with serious mental health conditions to build recovery, independence, and employability. Conduct psychosocial and vocational assessments; design and implement person-centered psychiatric rehabilitation plans that include skills training, supported employment/education, and community integration. Coordinate care with clinical providers and community resources and track outcomes to advance clients' recovery goals.
- • Monitor and document recovery and functional progress, providing early intervention and safety planning as needed.
- • Partner with clients to set recovery, wellness, and vocational goals and design individualized psychiatric rehabilitation plans.
- • Prepare and maintain clinical and rehabilitation records, including assessments, recovery plans, progress notes, eligibility, and service authorizations.
- • Arrange for psychiatric, psychological, vocational, and psychosocial evaluations to inform rehabilitation planning.
- • Synthesize interviews, records, and clinical input to assess functional strengths, barriers, and service eligibility.
- • Deliver recovery-oriented interventions that build coping skills, illness self-management, social functioning, and readiness for work or school, individually and in groups.
- • Provide ongoing support during supported employment or education, addressing symptoms at work or school and evaluating fit.
- • Build and maintain referral networks with community mental health centers, peer programs, schools, and employers.
- • Identify barriers such as stigma, symptom triggers, benefits concerns, housing or transportation issues, and develop strategies to overcome them.
- • Coordinate on-the-job coaching, reasonable accommodations, and cognitive aids to support success in work or school.
- • Collaborate with psychiatrists, therapists, case managers, and occupational therapists to implement integrated rehabilitation plans and support medication adherence through education and coordination.
- • Use standardized screening and assessment protocols to determine psychosocial, functional, and vocational needs.
- • Conduct job development and placement using evidence-based supported employment (e.g., IPS), and evaluate job matches, satisfaction, and retention.
- • Engage families and natural supports in education, relapse prevention, and achievement of social, residential, and employment goals.
- • Partner with community agencies to expand psychiatric rehabilitation, clubhouse, peer support, and wellness programs.
- • Manage case service funds and authorize payments for rehabilitation, training, transportation, or supports.
- • Supervise psychiatric rehabilitation staff and peer specialists, as assigned, and provide guidance and training.
