Description
Support social workers and other human services professionals in delivering client services across settings such as social work, psychology, rehabilitation, and family support. Help clients identify and obtain benefits, social services, and community resources. Assist with developing, organizing, and running programs to prevent and address issues related to substance use, relationships, rehabilitation, and dependent care.
  • • Provide information or refer individuals to public or private agencies or community services for assistance.
  • • Maintain case notes and prepare reports on client contacts for supervisors.
  • • Visit clients at home or attend group meetings to explain agency services, requirements, and procedures.
  • • Advise clients on benefits, child care, nutrition, budgeting, and household management.
  • • Submit reports and review cases or concerns with a supervisor.
  • • Facilitate or monitor daily group activities in residential or program settings.
  • • Interview clients and families to gather social, educational, legal, and health histories.
  • • Transport and accompany clients to appointments or essential services.
  • • Explain agency or program rules and procedures.
  • • Coach clients on safe, economical meal planning and preparation.
  • • Demonstrate safe use and care of household or program equipment.
  • • Consult with supervisors to tailor service plans for individuals and families.
  • • Promote healthy, cost-effective food choices.
  • • Orient clients to available onsite or community facilities and resources.
  • • Assist in locating housing for displaced or homeless individuals.
  • • Assist clients with completing forms, such as tax, benefits, or rent applications.
  • • Assist with household budgeting and basic financial planning.
  • • Assist in assessing clients' needs and strengths to inform interventions.
  • • Help implement behavioral management strategies and care plans.
