Description
Provide direct, developmentally informed care to children and adolescents in residential, school, and community settings. Implement individualized service plans, teach life and social skills, coordinate resources, and promote safety and well-being for youth and families. Collaborate with social workers and other providers to prevent and address challenges related to behavior, mental health, relationships, substance use, and caregiving.
- • Provide information and referrals to youth and families for school, health, and community services.
- • Conduct home, school, or residential visits and attend team meetings to coordinate care and explain expectations.
- • Coach youth in daily living skills, hygiene, budgeting, and household routines.
- • Submit timely reports and review concerns with a supervisor.
- • Lead and supervise daily group activities in residential or after-school settings, using de-escalation as needed.

- • Help arrange foster, kinship, or transitional living placements when needed.
- • Assist youth and families with school, program, and benefits paperwork.
- • Teach simple budgeting, saving, and spending plans.
- • Assess strengths, risks, and developmental needs to inform interventions.
- • Implement trauma-informed behavior supports and individualized service plans.
- • Maintain accurate case notes, incident reports, and progress summaries.
