Description
Support community-based programs by assisting social workers, program coordinators, and partner agencies with outreach, participant services, and daily operations. Help individuals and families access benefits and local resources; coordinate activities and workshops; and maintain records and reports. Contribute to prevention and support efforts related to health, housing, recovery, and family stability.
- • Maintain participant files, data entry, and routine reports for program staff and management, ensuring confidentiality.
- • Conduct outreach at community sites and group events to share services, requirements, and procedures.
- • Coach participants on accessing SNAP, childcare, transportation, and financial or housing resources.
- • Submit activity reports and review cases, barriers, and updates with a supervisor.
- • Help facilitate and monitor daily group activities, workshops, and classes.
- • Complete intake interviews to collect demographic, social, educational, and service histories.
- • Arrange transportation and, when necessary, accompany participants to appointments.
- • Explain program eligibility, participation guidelines, and site policies.
- • Support nutrition education sessions and healthy cooking demonstrations.
- • Set up rooms and materials and demonstrate safe use of program equipment.
- • Consult with supervisors and case managers to coordinate individualized service plans.
- • Promote economical, nutritious food choices and connect participants to food pantries or WIC/SNAP.
- • Orient participants to available facilities and resources, such as computer labs, childcare, or recreation areas.
- • Assist participants in locating housing or shelter and completing referrals.
- • Help participants complete applications and forms for benefits, services, and program enrollment.
- • Provide basic budgeting and money-management coaching and tools.
- • Conduct basic needs assessments and screenings to identify appropriate referrals and interventions.
- • Support implementation of participant goals and track progress, attendance, and outcomes.
- • Provide information and refer community members to appropriate programs, agencies, and services.
