Description
Provide peer-based support to individuals and families experiencing mental health or substance use challenges, using lived experience to foster hope, recovery, and self-advocacy. Help clients navigate benefits and community resources, coordinate with social and clinical providers, and assist with developing, organizing, and facilitating recovery-focused groups and programs that prevent relapse and promote wellness.
  • • Provide information and warm handoffs to community resources, benefits, and support services.
  • • Maintain accurate, timely documentation of contacts, goals, and progress.
  • • Conduct outreach and meet clients in homes or community settings to explain peer services.
  • • Coach clients on daily living, wellness, budgeting, and community navigation as requested.
  • • Review cases and seek supervision for complex issues, safety concerns, and professional growth.
  • • Facilitate peer-led support groups, workshops, and recovery activities.
  • • Accompany clients to appointments, court, or community meetings as an advocate or ally.
  • • Clarify program expectations, rights, responsibilities, and confidentiality.
  • • Teach and model coping skills, recovery planning (e.g., WRAP), and self-care practices.
  • • Orient clients to available program spaces, groups, and community activities.
  • • Collaborate with clinicians, case managers, and family (with consent) to align care plans.
  • • Support clients in identifying housing, employment, education, and social goals.
  • • Assist clients with applications and forms for benefits, services, housing, or transportation.
  • • Help clients create and follow personal budgets and resource plans.
  • • Work with clients to identify strengths, needs, and goals to guide support.
  • • Co-create and update individualized recovery and wellness/crisis plans with clients.
  • • Promote harm reduction, relapse prevention, and wellness strategies tailored to client goals.
  • • Recognize signs of crisis, use de-escalation skills, and connect clients to appropriate help.
  • • Uphold ethical peer support boundaries, cultural humility, and trauma-informed practices.
