Description
Champion and represent clients' interests within social and human service systems, ensuring access to benefits, protections, and community resources across areas such as mental health, rehabilitation, substance use, housing, and family support. Educate clients on rights and options, coordinate services with providers, and help prevent or resolve issues affecting well-being, relationships, and care.
- • Provide information and referrals to public agencies and community services.
- • Maintain accurate case records and prepare reports for supervisors or partners.
- • Conduct home visits and attend community meetings to explain services, rights, and procedures.
