Description
Assist case managers and social workers in coordinating and delivering client services across behavioral health, rehabilitation, and social services. Conduct intakes, help clients access benefits and community resources, schedule and track services, and provide follow-up through home or community visits. Maintain case records, prepare reports, support implementation of care plans, and ensure program and regulatory compliance.
  • • Provide information or refer clients to community resources, benefits, and services.
  • • Maintain case files and document contacts, services, and outcomes in agency systems.
  • • Conduct intake interviews and screenings to gather client histories and eligibility information.
  • • Schedule and coordinate client appointments, services, and case conferences.
  • • Assist clients in completing applications and forms, including benefits, housing, and healthcare.
  • • Educate clients on program requirements, policies, and procedures.
  • • Perform home visits or meet clients in the community to deliver support and follow up.
  • • Arrange and, when needed, provide transportation or accompaniment to appointments.
  • • Support case managers in developing and updating service or care plans.
  • • Monitor client progress and engagement; report changes and concerns to supervisors.
  • • Coordinate referrals and obtain releases of information to share records as appropriate.
  • • Assist with housing navigation and linkage to shelter or permanent housing resources.
  • • Provide coaching on basic life skills, budgeting, and resource utilization.
  • • Track benefits status and deadlines; remind clients of renewals and documentation needs.
  • • Prepare summaries and reports for supervisors and stakeholders.
  • • Participate in team meetings, case reviews, and multidisciplinary coordination.
  • • Maintain a current directory of community partners and program eligibility criteria.
  • • Support discharge planning and transitions of care, including aftercare follow-ups.
  • • Uphold confidentiality, informed consent, and compliance with agency and regulatory standards.
