Description
Provide direct, person-centered support to individuals with intellectual and developmental disabilities or behavioral health needs in home and community settings. Assist with daily living, skill building, community integration, and access to services; implement individualized support and behavior plans; ensure health and safety; advocate for client rights; and maintain accurate documentation and communication with the care team.
- • Provide information and referrals to community resources, benefits, and services.
- • Document services, progress notes, and incidents per agency and regulatory standards.
- • Provide in-home and community-based support and attend team meetings to coordinate care.
- • Coach clients in household management, personal hygiene, and accessing nutrition and childcare benefits.
- • Submit required reports and review concerns with supervisors.
- • Facilitate daily activities and community outings in residential or day program settings.
- • Gather intake information from clients and caregivers to inform individualized support plans.
- • Transport and accompany clients to appointments, employment, and community activities.
- • Explain and reinforce house rules, safety procedures, and clients' rights.
- • Support meal planning and preparation, promoting safe, healthy cooking skills.
- • Teach safe use of adaptive equipment and assistive technology.
- • Collaborate with supervisors and clinicians to implement Individualized Support Plans and behavior strategies.
- • Promote nutritious, cost-effective food choices during shopping and meals.
- • Maintain a clean, safe, and respectful living environment, following infection control and emergency procedures.
- • Assist with completing forms and applications for services, benefits, or employment.
- • Develop simple tools and routines to build financial and independent-living skills.
- • Monitor clients' health, behavior, and skills to adjust supports and seek clinical guidance.
- • Implement behavior support plans and crisis de-escalation techniques as trained.
- • Provide assistance with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.
- • Administer or prompt medications per training, physician orders, and agency policy, and track adherence.
