Description
Support counselors and mental health professionals by assisting with intake, screening, scheduling, documentation, and referrals. Help clients access benefits and community resources and support implementation of counseling and prevention programs addressing substance use, relationships, rehabilitation, and family needs, under supervision.
- • Provide information and referrals to community mental health and social services.
- • Maintain confidential client records and prepare progress notes for counselors.
- • Conduct intake interviews and screenings to gather psychosocial history and risk factors.
- • Schedule appointments, send reminders, and coordinate follow-up.
- • Explain program procedures, clinic policies, confidentiality, and consent to clients.
- • Assist clients with completing intake, insurance, and benefits paperwork.
- • Support group counseling sessions by setting up rooms, managing logistics, and tracking attendance.
- • Prepare psychoeducational materials and handouts on coping skills and wellness.
- • Observe client behavior and mood and promptly report significant changes to the counselor.
- • Consult with the supervising counselor to tailor service plans and discuss client concerns.
- • Arrange or accompany clients to appointments and community resources when needed.
- • Conduct outreach in homes, schools, or community settings to share service information.
- • Help clients navigate resources for housing, employment, education, childcare, or transportation.
- • Coordinate care with external providers and agencies as directed.
- • Enter data, submit routine reports, and review issues with the supervisor.
- • Support implementation of treatment and behavior plans under clinician direction.
- • Assist with crisis triage by alerting clinical staff and following safety protocols.
- • Maintain resource lists and update community service information.
