Description
Plan, implement, and evaluate medically approved therapeutic recreation programs for patients in hospitals, nursing homes, and other settings. Facilitate activities such as sports, outings, drama, social events, music, and crafts to improve functioning and quality of life. Assess patient status and recommend individualized recreation interventions in coordination with the care team.
- • Obtain information from medical records, staff, families, and patients to assess abilities, needs, and interests.
- • Develop individualized treatment plans with measurable recreation therapy goals.
- • Conduct therapy sessions to improve patients’ physical, cognitive, emotional, and social well-being.
- • Instruct patients in adaptive activities and techniques such as sports, dance, music, art, and relaxation.
- • Observe, analyze, and document participation, reactions, and progress; adjust plans as needed.
- • Encourage patients to build leisure skills and engage in health-promoting activities.
- • Plan, organize, and lead group and community-based activities to support rehabilitation and integration.
- • Collaborate with the interdisciplinary treatment team to plan and evaluate programs.
- • Prepare and submit progress notes, charts, and outcome reports to the care team.
- • Educate patients and caregivers on leisure resources and strategies for continued participation.
- • Develop discharge plans, including community referrals and home programs.
