Description
Provide in-home assessment and treatment for speech, language, voice, cognitive-communication, and swallowing disorders. Develop functional plans of care, train caregivers, and recommend AAC as needed. Collaborate with the home health team, ensure safety in the home setting, and maintain compliant documentation.
  • • Conduct home-based speech, language, voice, cognitive-communication, and swallowing evaluations using standardized tools.
  • • Assess home environment factors impacting communication and swallowing safety.
  • • Develop and implement individualized, functional treatment plans aligned with physician orders and agency policies.
  • • Monitor patient progress and modify goals, frequency, and interventions as needed.
  • • Train patients and caregivers in safe swallowing strategies, diet texture modifications, and mealtime positioning.
  • • Instruct in communication strategies, including cueing, compensatory techniques, and caregiver coaching.
  • • Teach oral-motor, breath support, and voice exercises to improve intelligibility and vocal quality.
  • • Create and progress home exercise programs and carryover activities.
  • • Recommend, trial, and train in AAC systems, low- and high-tech, and communication boards.
  • • Use mobile EMR to document evaluations, visit notes, goals, progress, charges, and discharge summaries accurately and timely.
  • • Participate in interdisciplinary care conferences and coordinate with nursing, PT, OT, social work, and physicians.
  • • Educate patients and families on condition, prognosis, and strategies to manage communication and cognitive changes.
  • • Address cognitive-communication needs related to medication management, safety awareness, and instrumental ADLs.
  • • Screen hearing and refer to audiology or ENT as indicated.
  • • Evaluate aspiration risk and collaborate with physicians and dietitians on plan of care.
  • • Employ therapeutic activities embedded in daily routines in the home setting.
  • • Utilize telepractice for follow-ups when appropriate and permitted.
  • • Adhere to infection control, fall prevention, and emergency procedures during home visits.
  • • Manage scheduling, travel, and caseload independently while meeting visit productivity and timeliness standards.
  • • Consult and refer patients to additional medical, community, or caregiver support services.
  • • Supervise and mentor speech-language pathology assistants per state and agency guidelines.
  • • Participate in continuing education and in-service training to maintain competencies and licensure.
