Organize, maintain, and safeguard patient health records in paper and electronic systems, ensuring accuracy, completeness, and confidentiality. Duties include assembling charts, scanning and indexing documents, updating EHR data, processing record requests, and routing clinical information to authorized parties.
  • • Assemble and prepare patient charts for visits, procedures, or admissions.
  • • Scan, index, and file medical documents into the electronic health record (EHR).
  • • Verify document accuracy, patient identifiers, and record placement.
  • • Receive, route, and upload lab results and clinical reports to the correct chart.
  • • Retrieve, file, and track charts to support clinicians and care teams.
  • • Process and document requests for medical records from patients and authorized third parties.
  • • Verify identity, consents, and legal authority before releasing information.
  • • Release and transmit records via mail, secure email, fax, or patient portal per HIPAA and policy.
  • • Audit charts for completeness and obtain missing signatures or documents from providers.
  • • Maintain, archive, and purge paper records; coordinate off-site storage and retrievals.
  • • Operate scanners and health information systems to update records and run work queues or reports.
  • • Maintain disclosure logs, request logs, and chart movement tracking.
  • • Merge duplicate records and correct demographic or charting errors per policy.
  • • Prepare routine correspondence regarding record requests, amendments, or denials.
  • • Order and maintain forms and supplies for medical records operations.
