Description
Plan, deliver, and evaluate health education initiatives that help individuals, families, and communities adopt and maintain healthy lifestyles. Use data to assess community needs and to plan, implement, monitor, and evaluate programs, policies, and environments that promote health. Coordinate among health systems, providers, insurers, and patients to address individual and population health needs. Serve as a resource to the public and health professionals, and may administer budgets or grants for health education activities.
  • • Document program activities, outputs, and outcomes, including applications, presentations, and individuals served.
  • • Plan, develop, and deliver health education and promotion programs (workshops, conferences, school/community sessions).
  • • Build and sustain partnerships with agencies and organizations involved in public health.
  • • Create operational plans and procedures to meet health education objectives.
  • • Collaborate with health professionals and community groups to assess needs, map services, and set goals.
  • • Maintain program databases, contact lists, communication networks, and other information systems.
  • • Supervise or coordinate staff and volunteers to implement health programs and objectives.
  • • Design and conduct evaluations and studies to measure program quality, reach, and impact.
  • • Share program information via press releases, media campaigns, and program websites or social channels.
  • • Prepare and manage grant applications and reporting to secure and steward funding.
  • • Advise agencies and organizations on needs assessment and education program design and delivery.
  • • Curate and maintain health education resource libraries for staff and community partners.
  • • Conduct or coordinate community health needs assessments and public health surveys.
  • • Develop educational materials and curricula for community groups and for local and state agencies.
  • • Produce and distribute health education materials (reports, bulletins, visual aids) on topics such as tobacco, vaccination, and chronic disease prevention.
  • • Design and facilitate training for new staff and continuing education for existing staff.
