Description
Provide and manage diabetes self-management education and support (DSMES) programs to help individuals and families prevent complications and sustain healthy lifestyles. Use clinical and population data to identify needs before planning, implementing, monitoring, and evaluating diabetes education, policies, and environments. Link health systems, providers, insurers, and patients to coordinate diabetes care and resources. Serve as a diabetes resource for individuals, clinicians, and the community, and may administer budgets and billing for DSMES services.
  • • Document DSMES encounters, care plans, glucose data, outcomes, referrals, and attendance.
  • • Develop and deliver diabetes education workshops, group classes, and one-to-one coaching, in person and via telehealth.
  • • Build and maintain referral pathways with primary care, endocrinology, pharmacies, and community partners.
  • • Create operational protocols for DSMES delivery, insulin teaching, CGM/pump onboarding, and follow-up.
  • • Collaborate with clinicians and community groups to assess diabetes needs, map services, and set measurable goals.
  • • Maintain patient registries, EHR templates, contact lists, and communication tools to support program operations.
  • • Supervise and mentor education staff, peer educators, and volunteers implementing diabetes programs.
  • • Design and conduct program evaluations tracking A1C, hypoglycemia, hospitalization, and participant satisfaction.
  • • Promote diabetes programs through outreach, social media, newsletters, and program web pages.
  • • Prepare and manage grants and reimbursement processes to fund DSMES and related initiatives.
  • • Advise clinics and agencies on assessing diabetes education needs and integrating DSMES into care workflows.
  • • Curate and update multilingual diabetes education resources, toolkits, and technology guides.
  • • Conduct community assessments and surveys on diabetes prevalence, risk factors, and barriers to care.
  • • Develop tailored curricula and materials for schools, worksites, faith groups, and prenatal programs.
  • • Produce and distribute materials on glucose monitoring, nutrition, physical activity, foot care, sick-day rules, vaccines, and smoking cessation.
  • • Train staff on diabetes basics, insulin administration, hypoglycemia treatment, and use of CGM and insulin pumps.
