Description
Provide culturally responsive outreach, health education, and care coordination to help individuals, families, and communities achieve and maintain healthy lifestyles. Identify needs through outreach and data collection; connect clients to clinical and social services; support navigation, adherence, and follow-up; and promote healthy behaviors, policies, and environments. Serve as a trusted liaison and advocate between residents, health systems, insurers, and public agencies.
- • Document outreach, encounters, referrals, services provided, and outcomes in logs or electronic systems.
- • Conduct home visits, community outreach, and group sessions to deliver health education and support behavior change.
- • Build and maintain relationships with clinics, social service agencies, schools, and community organizations to coordinate care and resources.
- • Assist in implementing program activities and workflows to meet community health objectives.
- • Collaborate with healthcare providers and community leaders to identify barriers, priorities, and goals for addressing needs.
- • Maintain up-to-date resource directories, contact lists, and client tracking systems to support service navigation.
- • Support clients with appointment scheduling, insurance or benefits enrollment, transportation, and other social needs.
- • Collect screenings, surveys, and client feedback to monitor progress and program performance.
- • Share program information through presentations, community events, and approved media or social channels.
- • Assist with preparing reports and documentation for program monitoring and funding requirements.
- • Provide input on culturally and linguistically appropriate health messages and materials.
- • Distribute and explain health education materials on topics such as vaccines, chronic disease, maternal-child health, and prevention.
- • Facilitate or co-facilitate peer support groups, workshops, and classes.
- • Perform care coordination, including warm handoffs and follow-up to ensure service uptake and medication adherence.
- • Advocate for clients, identify systemic barriers, and relay community concerns to program staff and partners.
- • Participate in training and continuing education; may mentor new CHWs or volunteers.
