Description
Partner with individuals and families to set health goals, build self-management skills, and sustain healthy lifestyles. Use assessments and data to tailor coaching plans; implement, monitor, and evaluate behavior change interventions across in-person and virtual settings. Coordinate with clinicians, care managers, and community resources to address individual and population health needs. Serve as an accessible resource for health information, navigation, and motivation while tracking outcomes and managing caseload priorities.
- • Document coaching sessions, goals, care plans, outcomes, and referrals.
- • Conduct health risk assessments and readiness-to-change screenings.
- • Develop personalized coaching plans for nutrition, activity, sleep, stress, and medication adherence.
- • Deliver one-on-one and group coaching via phone, video, and in person.
- • Use motivational interviewing and evidence-based behavior change techniques.
- • Monitor client progress, adjust plans, and escalate clinical concerns as needed.
- • Coordinate with primary care, specialists, care managers, and community programs.
- • Maintain secure client records, registries, and communication logs.
- • Track and report program metrics, satisfaction, and quality outcomes.
- • Create and distribute educational materials and digital resources.
- • Support outreach, recruitment, and enrollment of eligible participants.
