Description
Provide and manage diabetes self-management education and support (DSMES) programs to help individuals and families prevent complications and sustain healthy lifestyles. Use clinical and population data to identify needs before planning, implementing, monitoring, and evaluating diabetes education, policies, and environments. Link health systems, providers, insurers, and patients to coordinate diabetes care and resources. Serve as a diabetes resource for individuals, clinicians, and the community, and may administer budgets and billing for DSMES services.
