Description
Assess eligibility and enroll individuals in government assistance programs by verifying documentation, explaining requirements and options, and processing applications, updates, and terminations.
  • • Answer applicants' questions about program options, eligibility, and enrollment procedures.
  • • Conduct initial and periodic enrollment and recertification interviews to confirm continued eligibility.
  • • Explain eligibility criteria, required documentation, enrollment steps, coverage start dates, and participant rights and responsibilities.
  • • Process approvals, changes, and terminations of enrollments, and refer individuals to other programs or resources as needed.
  • • Collect, verify, and record personal, financial, and residency information to determine enrollment eligibility.
  • • Interview applicants to gather information needed to complete enrollment applications.
  • • Contact employers, agencies, or third parties to verify income, employment, and other eligibility factors.
  • • Maintain accurate case files and enrollment records, and prepare required reports.
  • • Schedule enrollment appointments, orientations, and recertification reviews.
  • • Assist applicants in completing enrollment forms, online submissions, and supporting paperwork.
  • • Route applicants to case managers, counselors, or partner services per program guidelines.
  • • Share relevant enrollment and eligibility information with caseworkers and program staff.
  • • Determine benefit levels or service tiers based on eligibility rules and authorize enrollments in approved programs.
  • • Monitor enrollment status, effective dates, and changes that may affect benefits.
  • • Provide application assistance for related benefits or services as permitted by program policy.
  • • Flag and investigate potential discrepancies or fraud in enrollment data and documentation.
  • • Conduct periodic eligibility reviews and, when required, home or site visits to ensure compliance with program requirements.
