Description
Counsel and support individuals with alcohol, opioid, tobacco, and other substance use disorders. Provide individual and group therapy, involve families, develop and adjust treatment plans, coordinate care, and support prevention and long-term recovery.
- • Counsel clients individually or in groups to reduce substance use, build coping skills, and support recovery.
- • Complete and maintain accurate assessments, progress notes, and required reports.
- • Develop individualized treatment plans using evidence-based practices and client histories.
- • Review and evaluate client progress against measurable goals in treatment and care plans.
- • Interview clients, review records, and consult with other professionals to assess substance use and co-occurring conditions.
- • Provide crisis intervention and advocate for clients during emergencies.
- • Educate clients and families on addiction, recovery, medications, and available services; make referrals as needed.
- • Modify treatment plans to reflect changes in client status or motivation.
- • Coordinate counseling and care with physicians, nurses, psychiatrists, and social workers.
- • Attend trainings and supervision to maintain and enhance clinical skills.
- • Plan and implement discharge, relapse prevention, and aftercare programs.
- • Conduct program orientation and intake for new clients.
- • Counsel family members to help them support clients in recovery.
- • Participate in case conferences and staff meetings.
- • Act as liaison between clients and medical staff for detoxification, MAT, and lab testing.
- • Coordinate with courts, probation officers, and community services as required.
- • With consent, update family or supports on treatment planning and progress.
- • Instruct staff or peers in program methods, procedures, and group facilitation.
- • Follow up with discharged clients to evaluate treatment effectiveness and support continuity of care.
- • Develop, implement, or evaluate community prevention and recovery education programs.
- • Supervise or direct interns or support staff providing client services.
- • Assess substance use via urine drug screens or other toxicology tests per protocol.
