Description
Counsel and support incarcerated or justice-involved individuals with substance use disorders in jails, prisons, or community corrections. Provide assessment, evidence-based individual and group treatment, crisis intervention, and reentry planning while coordinating with custody, medical, mental health, and legal stakeholders to reduce relapse and recidivism.
- • Counsel clients individually and in groups to address substance use and criminal risk.
- • Maintain secure, accurate clinical and legal records per facility and regulatory standards.
- • Develop individualized treatment plans based on assessments, history, and custody considerations.
- • Review progress against measurable goals and update treatment and reentry plans.
- • Conduct intake screenings and biopsychosocial assessments; review records and consult with staff.
- • Provide crisis intervention and advocate for client needs within security protocols.
- • Educate clients on addiction, recovery, overdose prevention, and available services; make referrals.
- • Modify treatment to reflect changes in housing, sanctions, or clinical status.
- • Coordinate care with medical, mental health, and custody staff, including support for MAT.
- • Attend required security and clinical trainings to maintain competencies.
- • Plan and implement reentry and aftercare, linking clients to community providers and supervision.
- • Lead program orientation and evidence-based groups such as CBT and relapse prevention.
- • Engage approved family or support persons in education and reentry planning when permitted.
- • Participate in multidisciplinary case conferences and custody briefings.
- • Serve as liaison between clients and healthcare providers and correctional staff.
- • Prepare reports for courts, probation, parole, or boards and coordinate compliance requirements.
- • Document and report critical incidents and threats of harm per facility policy.
- • Train or mentor interns, peer mentors, or new staff in program procedures.
- • Track post-release outcomes and adjust program approaches based on results.
- • Deliver prevention and health promotion initiatives within the facility.
- • Supervise or direct program aides or peer facilitators as assigned.
- • Conduct or coordinate drug testing and interpret results in accordance with policy.
- • Ensure adherence to DOC policies, HIPAA, PREA, and professional ethics.
