Description
Provide individual, group, and family counseling in an outpatient setting to address substance use and co-occurring behavioral disorders; conduct assessments and develop treatment plans, coordinate care with healthcare and community providers, deliver psychoeducation and relapse-prevention services, and support recovery through evidence-based interventions and referrals.
- • Provide individual and group counseling to help clients reduce substance use, build coping skills, and make life changes in an outpatient setting.
- • Maintain accurate, timely electronic health record documentation of assessments, progress notes, treatment, referrals, and outcomes.
- • Complete biopsychosocial assessments and develop person-centered, evidence-based treatment plans.
- • Monitor and evaluate client progress toward measurable goals and update care plans as needed.
- • Conduct intake interviews, review records, and collaborate with clinicians to determine eligibility and level of care.
- • Provide crisis intervention, safety planning, and advocacy to resolve urgent issues.
- • Deliver psychoeducation to clients and families on addiction, mental health, recovery resources, and available services; make referrals as needed.
- • Adjust treatment frequency and interventions based on changes in clinical status, risk, and engagement.
- • Coordinate care with psychiatrists, primary care providers, nurses, case managers, and social workers.
- • Attend supervision and training to maintain competencies, credentials, and best practices.
- • Plan and implement discharge, relapse-prevention, and aftercare services; connect clients to community supports.
- • Conduct orientation sessions for new clients, explaining program expectations, schedules, and informed consent.
- • Provide family counseling to enhance understanding, communication, and support for clients.
- • Participate in multidisciplinary case conferences and staff meetings to review cases and align care.
- • Serve as a liaison between clients and medical providers regarding appointments, medications, and treatment recommendations.
- • Coordinate with courts, probation or parole, child welfare, and community services to support treatment compliance when applicable.
- • With client consent, communicate treatment plans and progress to family or significant supports.
- • Instruct support staff or peers in program procedures, group facilitation, and documentation standards, as assigned.
- • Follow up with discharged clients to assess outcomes and the effectiveness of treatment.
- • Assist with clinic-based prevention, outreach, and health promotion activities.
- • Provide guidance or oversight to peers or support staff delivering client services, as assigned.
- • Administer or coordinate toxicology screens per program protocol and interpret results within scope.
- • Train or supervise interns or newly hired staff under appropriate licensure and supervision rules.
