Description
Assess, support, and empower individuals, families, and communities facing social, behavioral, or health challenges. Services may include psychosocial assessment, individual and group counseling, crisis intervention, case management, advocacy, prevention, resource linkage, and education.
- • Counsel clients in individual or group sessions to address psychosocial, behavioral, health, or economic challenges.
- • Collaborate with interdisciplinary teams to plan and coordinate services based on client needs.
- • Monitor, evaluate, and document client progress toward service or treatment goals.
- • Refer clients and families to community resources for housing, healthcare, benefits, employment, or specialized treatment, and follow up to ensure access and effectiveness.
- • Support and educate family members to enhance their understanding and ability to assist the client.
- • Modify service or care plans in response to changes in client status or goals.
- • Develop, implement, or facilitate prevention and wellness programs and community outreach.
- • Supervise, mentor, or coordinate staff, interns, or volunteers providing client services.
- • Participate in policy development, advocacy, or community organizing to address systemic barriers.
- • Conduct intake interviews, biopsychosocial assessments, and record reviews; consult with other professionals to evaluate client needs and risks.
- • Educate clients and communities about rights, services, coping strategies, health, safety, and available resources.
- • Assist clients with navigating systems and logistics, such as appointments, benefits applications, transportation, or supportive services.
- • Maintain professional competence through continuing education, supervision, research, and review of current literature.
