Description
Assess and treat individuals with substance use disorders and co-occurring mental health issues. Provide individual and group counseling, treatment planning, relapse prevention, crisis intervention, case management, family education, and recovery support.
- • Provide individual and group counseling for substance use and co-occurring disorders.
- • Coordinate care with physicians, nurses, psychiatrists, and peer staff, including medication-assisted treatment (MAT).
- • Conduct intake screenings and biopsychosocial assessments using DSM-5/ASAM criteria.
- • Develop individualized, measurable treatment plans using evidence-based practices.
- • Monitor and document client progress and outcomes per agency and regulatory standards.
- • Facilitate relapse-prevention, psychoeducation, and recovery skills groups.
- • Provide crisis intervention and safety planning; arrange higher levels of care when needed.
- • Refer clients to detox, medication-assisted treatment (MAT), residential, outpatient, housing, and other resources; follow up on service efficacy.
- • Educate and counsel families to support recovery and reduce stigma.
- • Revise treatment and recovery plans based on progress or changes in level-of-care needs.
- • Assist clients with appointments, transportation, and aftercare coordination to support adherence.
- • Maintain confidentiality and documentation compliance (HIPAA, 42 CFR Part 2).
- • Engage in professional development through supervision and continuing education.
