Description
Coordinate and manage care for individuals with mental health and substance use disorders by assessing needs, developing service plans, linking to resources, advocating for benefits, and monitoring outcomes. Provide crisis response, psychoeducation, and care coordination across providers to support recovery and stability.
- • Conduct intake interviews and biopsychosocial assessments to determine client needs and risks.
- • Develop individualized service plans with measurable goals in collaboration with clients and providers.
- • Coordinate and schedule services across behavioral health, medical, housing, and social supports.
- • Refer clients and families to community resources and follow up to ensure engagement and effectiveness.
- • Monitor, document, and evaluate client progress toward goals; adjust plans as needed.
- • Facilitate case conferences and collaborate with clinicians, physicians, and community partners.
- • Provide brief counseling, psychoeducation, and skills coaching within scope of practice.
- • Assist clients with benefits, insurance authorizations, and eligibility for assistance programs.
- • Support clients in appointment adherence, transportation, and treatment compliance.
- • Advocate for client rights and access to services; address barriers to care.
- • Respond to crises, complete safety plans, and coordinate higher levels of care when indicated.
- • Maintain accurate, timely records and reports in compliance with agency and regulatory standards.
