Description
Provide assessment, psychotherapy, and case management to individuals, families, and groups facing mental health, behavioral, or co-occurring conditions. Deliver evidence-based treatment, crisis intervention, care coordination, advocacy, prevention, and education across healthcare and community settings.
  • • Provide individual, family, and group psychotherapy for mental health and co-occurring concerns.
  • • Conduct comprehensive biopsychosocial assessments, risk evaluations, and formulate clinical impressions and treatment recommendations.
  • • Collaborate with interdisciplinary teams to develop and coordinate treatment plans.
  • • Deliver crisis intervention and safety planning; de-escalate and stabilize acute situations.
  • • Monitor, evaluate, and document client progress toward goals using measurable outcomes.
  • • Revise treatment plans based on ongoing assessment and client response.
  • • Coordinate referrals and link clients and families to community and healthcare resources; follow up for effectiveness.
  • • Provide psychoeducation and support to families and caregivers.
  • • Support adherence to treatment plans, including appointment coordination and care navigation.
  • • Advocate for client rights and access to services; contribute to program or policy improvements.
  • • Develop and deliver prevention, wellness, and psychoeducational programs in clinical or community settings.
  • • Provide clinical supervision or consultation to social work staff or interns, as appropriate.
  • • Maintain professional competence through supervision, licensure requirements, and continuing education grounded in evidence-based practice.
