Description
Provide assessment, counseling, crisis intervention, and case management for individuals and families with mental health and co-occurring substance use needs in community settings. Deliver person-centered, trauma-informed care, coordinate services with community providers and resources, and lead outreach, prevention, advocacy, and education to improve access and recovery.
- • Conduct outreach, intake, and biopsychosocial assessments in clinics, homes, and community settings.
- • Provide individual and group counseling using evidence-based, trauma-informed, and culturally responsive approaches.
- • Perform crisis assessment, de-escalation, and safety planning; coordinate mobile or emergency services as needed.
- • Develop person-centered treatment and recovery plans addressing mental health, co-occurring substance use, and social needs.
- • Monitor, document, and evaluate progress toward goals in the EHR; adjust interventions based on outcomes.
- • Coordinate care and warm handoffs with psychiatrists, primary care, counselors, schools, shelters, and justice or child welfare partners.
- • Link clients and families to housing, benefits, transportation, employment, and specialty treatment; follow up to ensure access.
- • Engage and coach family and support systems to improve understanding, communication, and recovery support.
- • Facilitate psychoeducation, support groups, and community prevention or stigma-reduction activities.
- • Conduct home or field visits to reduce barriers and promote engagement and adherence.
- • Advocate for clients’ rights and contribute to community resource mapping, program development, and quality improvement.
- • Mentor or provide task supervision to peer support workers, case aides, or student interns as assigned.
- • Maintain compliance with agency policies, ethical standards, confidentiality, and required training and supervision.
