Description
Provide social services and support to individuals, families, and communities to enhance psychosocial functioning, safety, and quality of life. Assess needs, counsel, develop service plans, connect clients to resources, coordinate care, advocate for rights, and monitor outcomes across healthcare, behavioral health, child welfare, and community settings.
  • • Interview clients individually, with families, or in groups to assess needs, strengths, risks, and barriers.
  • • Provide counseling and psychoeducation on mental health, substance use, trauma, illness, poverty, housing, employment, and adjustment.
  • • Maintain confidential case records and prepare required reports and documentation.
  • • Develop, implement, and review individualized service or treatment plans with clients, performing follow-up and reassessment.
  • • Refer and link clients to community resources such as housing, food, employment, legal aid, medical and behavioral health care, and financial assistance, providing navigation support.
  • • Coordinate care and serve as a liaison among clients, families, providers, schools, courts, and community agencies.
  • • Conduct risk and safety assessments, create crisis plans, and provide crisis intervention and mandated reporting.
  • • Facilitate group counseling or support groups addressing grief, stress, recovery, parenting, or similar needs.
  • • Arrange and coordinate medical, psychiatric, psychological, or functional evaluations and use results to guide services.
  • • Advocate for client rights and access to services, assisting with benefits applications and eligibility.
  • • Educate clients and caregivers on coping skills, resources, and system navigation.
  • • Address legal and ethical issues, including abuse, neglect, domestic violence, and confidentiality, preparing documentation and testifying when needed.
  • • Conduct home, school, community, or hospital visits to assess environments and support engagement.
  • • Collaborate with multidisciplinary teams and participate in case conferences and discharge planning.
  • • Monitor outcomes and service quality, adjusting interventions based on evidence and client feedback.
  • • Participate in community outreach, program development, and policy advocacy on client and systemic issues.
  • • Ensure compliance with agency policies, regulations, and payer requirements, managing caseloads and deadlines.
  • • Collect and analyze collateral information such as medical records, employment history, school reports, or legal documents.
  • • Determine client eligibility for financial assistance or public benefits.
  • • Conduct needs assessments or social research to inform services and programs.
  • • Recommend placement in shelters, residential treatment, rehabilitation, or foster or adoptive care when appropriate.
  • • Supervise or mentor interns or junior staff when assigned.
