Description
Provide family-centered early intervention services and service coordination for infants and toddlers with developmental delays or disabilities and their families. Conduct screenings and evaluations, develop and monitor IFSPs, coach caregivers in natural environments, coordinate therapies and community resources, and support transition to preschool services in accordance with IDEA Part C.
- • Educate families on rights, procedural safeguards, and program eligibility under IDEA Part C; determine eligibility as required.
- • Lead caregiver support groups or workshops on child development, behavior, and routines.
- • Refer families to resources for housing, food, transportation, childcare, financial aid, legal assistance, and employment.
- • Participate in interagency meetings, community outreach, and Child Find activities to identify eligible children.
- • Collect and analyze service and outcome data to support quality improvement and reporting.
- • Train and mentor early intervention aides or new staff; provide peer consultation.
- • Advocate for culturally and linguistically responsive and accessible services for families.
- • Ensure confidentiality and ethical practice; comply with Medicaid or insurance billing and documentation standards.
- • Screen and assess infants and toddlers using standardized tools to identify developmental strengths and needs.
- • Interview caregivers and observe children in home and community settings to determine priorities and supports.
- • Develop, implement, and review Individualized Family Service Plans (IFSPs) with families and multidisciplinary teams.
- • Coach and educate caregivers to embed developmentally appropriate strategies within daily routines.
- • Coordinate and refer for services such as OT, PT, speech-language therapy, audiology, vision, mental health, and medical care.
- • Monitor child progress and family outcomes; adjust strategies based on data and feedback.
- • Maintain accurate case records, consents, and compliance documentation; prepare required reports.
- • Collaborate with pediatricians, therapists, child care providers, and school district personnel to align goals.
- • Plan and facilitate transitions to preschool or school-based services, including Part B referrals and meetings.
- • Conduct home and community visits to model interventions and connect families to resources.
- • Provide crisis support and complete mandated reporting for suspected abuse or neglect per policy.
- • Arrange hearing, vision, developmental, and other evaluations; coordinate multidisciplinary assessments.
