Description
Dispense and manage medications within a hospital, verifying provider orders and ensuring safe, effective use across inpatient and outpatient settings. Collaborate with the care team to optimize therapy, provide medication information, and oversee sterile compounding, dosing, and monitoring in line with hospital policies and regulations.
- • Verify medication orders in the EHR/CPOE for accuracy, appropriateness, dosing, route, and allergies.
- • Provide drug information to physicians, nurses, and patients on interactions, adverse effects, dosing, and storage.
- • Participate in patient care rounds to optimize regimens and monitor effectiveness and safety.
- • Perform therapeutic drug monitoring and adjust doses based on labs, organ function, and pharmacokinetics.
- • Compound and dispense sterile and nonsterile medications, including IV admixtures, TPN, and chemotherapy, per USP standards.
- • Prepare and check medications for operating rooms, emergency departments, and inpatient units.
- • Conduct medication reconciliation at admission, transfer, and discharge.
- • Educate patients and caregivers on discharge medications and self-administration.
- • Manage the formulary and support the P&T committee on medication use policies.
- • Perform medication-use evaluations and analyze prescribing trends to improve safety and compliance.
- • Maintain records for patient profiles, inventories, controlled substances, and regulatory audits.
- • Oversee automated dispensing cabinets, crash carts, and medication kits, ensuring proper stocking and security.
- • Implement and maintain procedures for mixing, labeling, storage, and disposal to meet legal and hospital requirements.
- • Collaborate with antimicrobial stewardship and other clinical programs to promote appropriate use.
- • Respond to codes and emergencies, preparing and verifying stat medications.
- • Supervise and mentor pharmacy technicians, interns, and residents.
- • Coordinate procurement and manage inventory of pharmaceuticals and supplies, addressing shortages and alternatives.
- • Support investigational drug services and clinical trials per protocol.
- • Troubleshoot and update pharmacy information systems, order sets, and decision support.
- • Ensure accurate charge capture and documentation for medications.
