Description
Clinical Pharmacists optimize medication therapy and patient outcomes through direct clinical care and collaboration with medical teams in hospitals and clinics. They verify and manage medication orders, provide evidence-based drug information, adjust dosing based on patient factors and monitoring, and lead safety, stewardship, and protocol initiatives.
- • Verify medication orders for accuracy, appropriateness, dosing, interactions, and allergies.
- • Perform medication reconciliation at admission, transfer, and discharge.
- • Round with providers to recommend and adjust pharmacotherapy plans.
- • Provide drug information on interactions, side effects, dosage, and monitoring.
- • Manage therapeutic drug monitoring and pharmacokinetic dosing for high-risk medications.
- • Implement antimicrobial stewardship and evaluate antibiotic use.
- • Assess prescribing trends to promote safety, adherence, and cost-effective use.
- • Prepare or verify sterile and hazardous compounded products per policy.
- • Develop and maintain evidence-based protocols, order sets, and pathways.
- • Monitor and report adverse drug events and conduct medication use evaluations.
- • Counsel patients and caregivers on medication use, devices, and storage.
- • Coordinate transitions of care and ensure discharge medication accuracy.
- • Document clinical interventions and maintain complete medication histories in the EHR.
- • Educate and precept pharmacy students, residents, and other clinicians.
- • Participate in P&T activities and formulary management.
- • Support emergency responses by providing rapid dosing and product selection.
- • Collaborate with case management to resolve coverage or prior authorization barriers.
- • Ensure compliance with regulatory, accreditation, and medication safety standards.
- • Contribute to quality improvement projects, audits, and research.
- • Optimize and troubleshoot pharmacy information systems and clinical decision support.
