Description
Assist patients, families, or health plan members with personalized non-clinical services, coordinating appointments, referrals, transportation, wellness resources, and logistics to simplify care navigation and enhance the healthcare experience.
- • Provide information on local and virtual healthcare resources, such as clinics, pharmacies, urgent care, and wellness programs.
- • Arrange transportation for medical visits, procedures, rehabilitation, or home-care services.
- • Receive, track, and deliver medical documents, test kits, prescriptions, or durable medical equipment.
- • Perform front-desk and administrative support, including intake, scheduling, and updating records.
- • Coordinate prescription pickup, refills, and delivery for patients or caregivers.
- • Handle special requests, such as locating rare medications or expediting specialist consultations or second opinions.
- • Arrange replacement of lost prescriptions, medical devices, or immunization records.
- • Secure medical interpreters or ASL services for in-person and telehealth appointments.
