Description
Provide direct patient care across diverse medical and behavioral health needs under the direction of nurses, physicians, and other providers. Monitor and document patients' physical and emotional status, promptly reporting changes to the care team. Support treatment and rehabilitation plans, assist with daily living needs and patient education, coordinate services, and administer medications as authorized.
  • • Monitor patients' physical and emotional status and promptly report changes or concerns to the care team.
  • • Provide basic nursing and personal care to diverse patients, including those with chronic, cognitive, or developmental conditions.
  • • Engage patients therapeutically, reinforce care plans, and provide education, support, and encouragement.
  • • Obtain and document vital signs and other measurements using approved devices.
  • • Encourage participation in mobility, therapeutic, and recreational activities that promote recovery and independence.
  • • Collaborate with nurses, physicians, therapists, and care coordinators to carry out treatment and discharge plans.
  • • Teach strategies that promote wellness, self-management, and independence.
  • • Use de-escalation techniques and assist with safety interventions per policy to protect patients and staff.
  • • Assist patients with activities of daily living, hygiene, grooming, and maintaining a safe, clean environment.
  • • Administer medications as authorized, follow provider orders, and document per policy.
  • • Manage medication and supply stocking, labeling, and recordkeeping in accordance with procedures.
  • • Complete intake and admission processes, gather health histories, and reconcile medications.
  • • Facilitate patient education or support sessions as directed by the care team.
  • • Communicate with families or caregivers to coordinate visits, care plans, and discharge needs.
  • • Train and mentor new staff on unit workflows, safety, and patient care procedures.
  • • Escort or transport patients to tests, therapies, or medical appointments within or outside the facility.
