Description
Provide direct patient care across diverse medical and behavioral health needs under the direction of nurses, physicians, and other providers. Monitor and document patients' physical and emotional status, promptly reporting changes to the care team. Support treatment and rehabilitation plans, assist with daily living needs and patient education, coordinate services, and administer medications as authorized.

- • Manage medication and supply stocking, labeling, and recordkeeping in accordance with procedures.
- • Complete intake and admission processes, gather health histories, and reconcile medications.
- • Facilitate patient education or support sessions as directed by the care team.
- • Communicate with families or caregivers to coordinate visits, care plans, and discharge needs.
- • Train and mentor new staff on unit workflows, safety, and patient care procedures.
- • Escort or transport patients to tests, therapies, or medical appointments within or outside the facility.
