Description
Provide hands-on support to individuals with disabilities, behavioral health needs, or aging-related conditions in homes, community programs, or residential facilities under the direction of healthcare or social service professionals. Assist with daily living, community engagement, health monitoring, and appointments while promoting independence, safety, and approved de-escalation or safety interventions when needed.
- • Provide individuals with disabilities or behavioral health needs with routine personal care, emotional support, and rehabilitation assistance under supervision.
- • Collaborate with interdisciplinary teams, such as nurses, therapists, case managers, and social workers.
- • Orient individuals to home, program, or facility routines and expectations.
- • Organize and encourage participation in social, educational, vocational, or recreational activities.
- • Support health tasks per protocol, including medication reminders or assistance, vital sign checks, and specimen collection as directed.
- • Prepare, serve, and assist with meals, following dietary plans.
- • Use de-escalation techniques and approved safety interventions to prevent injury.
- • Assist with intake, gather basic information, and document orientation.
- • Assist with bathing, dressing, grooming, toileting, and hygiene, modeling skills as needed.
- • Engage clients in hobbies, exercise, and community outings.
- • Monitor adherence to care plans, supervision levels, and safety protocols.
- • Accompany individuals to medical, dental, employment, educational, shopping, religious, or recreational appointments and events.
- • Maintain a clean, safe living environment; sanitize equipment and spaces.
- • Monitor health and behavior, conduct safety checks, and promptly report concerns.
- • Document services, vital signs, behaviors, meals, progress notes, incidents, and care plans.
- • Complete administrative tasks, including scheduling, data entry, and maintaining service records.
- • Provide companionship, active listening, and encouragement to promote independence and well-being.
