Description
Plan and deliver rehabilitation and reentry services for incarcerated individuals. Conduct assessments, develop and manage treatment plans, coordinate programs and community services, monitor progress, and recommend actions on programming, parole eligibility, and release conditions, including education and employment requirements.
  • • Prepare and maintain case files for assigned inmates, documenting assessments, plans, and progress.
  • • Write timely reports and progress notes on inmate participation, behavior, and goal attainment.
  • • Explain institutional rules, program requirements, and conditions of parole or supervised release to inmates.
  • • Interview inmates to identify criminogenic needs, including substance use, anger management, and cognitive skills deficits.
  • • Gather background information from inmates, family members, staff, and community contacts to inform case plans.
  • • Develop individualized treatment and reentry plans with clear goals, objectives, and rules of conduct.
  • • Facilitate or coordinate evidence-based programs such as CBT, life skills, vocational readiness, and substance use treatment.
  • • Build liaisons with correctional staff, community providers, Residential Reentry Centers, and social service agencies.
  • • Arrange referrals for medical, mental health, and substance use services in accordance with assessments or orders.
  • • Assist inmates with detainers, interstate matters, records requests, and applications for identification and benefits.
  • • Plan postrelease services, including housing, employment, education, counseling, and prosocial activities.
  • • Assess suitability for parole, work release, and reentry center placement and submit recommendations to decision-making bodies.
  • • Prepare parole or release packets and present findings to parole boards, classification committees, or reentry councils.
  • • Conduct risk and needs assessments using validated tools to guide programming and case decisions.
  • • Monitor compliance with treatment plans and institutional rules and recommend corrective or disciplinary actions as needed.
  • • Participate in unit team meetings and case conferences to review progress and adjust plans.
  • • Identify appropriate in-facility work, education, or vocational assignments aligned with case goals.
  • • Develop and distribute resource guides on community services and supports.
  • • Coordinate and monitor drug and alcohol testing related to treatment compliance, as required.
  • • Maintain accurate data in case management systems and track program outcomes and performance metrics.
  • • Provide prerelease orientations and reentry workshops on life skills, employment readiness, and supervision expectations.
  • • Coordinate warm handoffs to parole or probation officers and community providers at release.
