Description
Coordinate and manage mental health services for individuals and families to promote stability, recovery, and wellness, with an emphasis on prevention and continuity of care. Assess needs, develop and monitor service plans, connect clients to resources, advocate across systems, and collaborate with providers to address mental health, substance use, and social determinants.
  • • Maintain confidentiality of all client records and communications.
  • • Build rapport and facilitate client-centered goal setting and needs identification.
  • • Coach clients on practical coping and self-management strategies.
  • • Maintain comprehensive case files, service plans, progress notes, and reports.
  • • Provide supportive case management contacts to reinforce recovery and daily functioning.
  • • Conduct intake and biopsychosocial assessments, including collateral information.
  • • Advocate for clients and coordinate needed services, especially during crises.
  • • Develop, implement, and regularly update individualized service plans with measurable goals.
  • • Monitor clients' mental status, functioning, and social determinants to identify changes in need.
  • • Coordinate care and exchange information with families, providers, schools, courts, and community partners, with consent.
  • • Link clients and families to community resources, benefits, and specialty services.
  • • Educate families and natural supports about conditions, resources, and ways to support the client.
  • • Track outcomes and barriers; adjust plans and referrals to improve progress.
  • • Coordinate access to housing, transportation, employment, and peer support services.
  • • Revise service strategies promptly in response to changes in client status or preferences.
  • • Stay current on community resources, eligibility criteria, and best practices in case management.
  • • Develop transition, discharge, and aftercare plans to support continuity of care.
  • • Maintain and update a resource directory; identify service gaps and advocate for solutions.
  • • Monitor medication adherence and side-effect concerns; coordinate with prescribers.
  • • Organize or support prevention, wellness, and outreach initiatives.
  • • Screen for suicide risk, violence risk, and other safety concerns; complete safety plans.
  • • Complete mandated forms, authorizations, and documentation to meet regulatory and payer requirements.
  • • Supervise or mentor case aides, peer specialists, or interns, as assigned.
  • • Coordinate or deliver trainings for staff or community partners on resources and care navigation.
  • • Perform crisis response and de-escalation; coordinate emergency services when needed.
  • • Collaborate on multidisciplinary teams to align assessments, care plans, and transitions.
  • • Use electronic health records and data systems to document services and report outcomes.
