Description
Champion patients' rights and access by guiding individuals and families through the healthcare system. Provide information, education, and emotional support; coordinate communication among providers; help navigate insurance, billing, and community resources; and address barriers to timely, equitable care. May manage cases across the continuum, facilitate care transitions, and support informed decision-making, including end-of-life options.
- • Collaborate with clinicians and care teams to assess patient needs and priorities.
- • Explain patient rights, consent, and treatment options in clear, culturally sensitive language.
- • Coordinate communication among patients, families, and providers to ensure shared decision-making.
- • Refer patients and caregivers to community resources for financial aid, transportation, housing, legal support, or education.
- • Provide education and emotional support to help patients adjust to diagnoses and treatment.
- • Facilitate support groups or family meetings to align goals of care.
- • Advocate to resolve access, safety, or quality-of-care issues and escalate concerns when necessary.
- • Identify social determinants and other barriers to care through interviews and record reviews.
- • Arrange appointments, authorizations, and transitions of care; track referrals and services.
- • Assist with insurance navigation, benefits verification, prior authorizations, appeals, and billing disputes.
- • Modify navigation and advocacy plans in response to changes in health status, coverage, or preferences.
- • Monitor and document patient progress, service utilization, and outcomes against care goals.
- • Educate staff and patients on grievance processes and service access pathways.
- • Collect and relay patient experience data to inform quality improvement and policy changes.
- • Coordinate language access, disability accommodations, and health literacy-appropriate materials.
- • Support discharge planning by clarifying instructions, medications, and follow-up care.
- • Discuss advance directives, palliative care, and hospice resources to support informed choices.
