Description
Provide mental health assessment, diagnosis, psychotherapy, and care coordination for individuals, families, and groups coping with behavioral health and health-related challenges. Deliver evidence-based counseling, crisis intervention, case management, and referrals; collaborate with medical and psychiatric teams to promote health, prevent relapse, and address barriers to accessing care.
- • Conduct comprehensive biopsychosocial and risk assessments.
- • Diagnose mental health and substance use disorders within licensure scope.
- • Provide individual, group, and family psychotherapy using evidence-based modalities.
- • Develop, implement, and update person-centered treatment plans.
- • Collaborate with physicians, psychiatrists, nurses, and other providers to coordinate care.
- • Deliver crisis assessment, de-escalation, and safety planning, including suicide risk management.
- • Refer clients and families to community resources for housing, benefits, legal aid, and social supports.
- • Facilitate psychoeducation and support groups and counsel family caregivers.
- • Advocate for clients to resolve barriers to care, benefits, and services.
- • Identify social and environmental factors affecting treatment through interviews and record review.
- • Use assessment and consultation data to plan, coordinate, and evaluate treatment and care transitions.
- • Monitor, evaluate, and document client progress toward measurable goals in the EHR.
- • Modify interventions and treatment plans in response to changes in client status.
- • Complete clinical documentation, utilization review, and insurance authorizations, including Medicaid/Medicare requirements.
- • Plan and coordinate discharge and aftercare from hospitals or programs.
- • Assess and report suspected abuse or neglect in accordance with legal and ethical standards.
- • Participate in supervision, mentor staff or interns, and contribute to program development and quality improvement.
