Description
Identify and notify people exposed to infectious diseases, conduct interviews to elicit contacts, and provide guidance on testing, isolation, and prevention. Act as a liaison by connecting clients to healthcare, testing, vaccination, and social supports to enable safe isolation or treatment. Maintain confidential records and use public health systems to track cases, contacts, and outcomes. Collaborate with partners to improve service access and response. May assist with outbreak investigations, community outreach, and data collection to inform public health actions.
