Description
Coordinate, implement, and evaluate community health programs that promote healthy behaviors and improve access to care. Serve as a liaison among residents, healthcare providers, and social service agencies while advocating for community health priorities. Supervise outreach efforts, organize preventive services and education, manage data and resources, and ensure cultural competence and regulatory compliance.
  • • Coordinate risk‑reduction programming for lead exposure, HIV, prenatal substance use, and domestic violence, with appropriate referrals.
  • • Promote sanitation and hygiene initiatives through culturally appropriate materials and trainings.
  • • Develop self‑management education curricula for conditions such as diabetes and hypertension.
  • • Lead parental outreach to improve understanding of childhood immunizations and access to services.
  • • Advocate for community health priorities with health systems and government agencies.
  • • Represent the program at community meetings, coalitions, and health fairs to build partnerships.
  • • Establish protocols for home visits and oversee staff conducting visits to high‑risk clients.
  • • Implement reminder and follow‑up systems to ensure clients complete recommended actions.
  • • Coordinate development and distribution of flyers, brochures, and digital content for targeted audiences.
  • • Identify and prioritize high‑risk or underserved groups using data and stakeholder input.
  • • Conduct community health needs assessments to identify service gaps.
  • • Ensure availability of interpretation, translation, and cultural mediation services.
  • • Maintain program data systems and ensure accurate, timely client and service documentation.
  • • Develop nutrition support initiatives for children, older adults, and other high‑risk groups.
  • • Coordinate community screening events and ensure quality and data capture.
  • • Collect and analyze community feedback to improve service accessibility and acceptability.
  • • Build and manage referral networks; track referrals and outcomes.
  • • Organize and facilitate classes and workshops consistent with cultural norms.
  • • Coordinate assistance for families applying for Medicaid, WIC, or related benefits through partner agencies.
  • • Oversee collection of vital statistics and program metrics; prepare dashboards and reports.
  • • Develop individualized and group care plans and goal‑setting tools; monitor progress.
  • • Ensure compliance with mandatory reporting of abuse, neglect, or threats of harm.
  • • Arrange transportation support and appointment accompaniment protocols with staff or volunteers.
  • • Train, coach, and supervise community health workers and volunteers; manage schedules and workloads.
  • • Prepare grant proposals, budgets, and funder reports; manage program resources.
  • • Ensure compliance with privacy, safety, and regulatory requirements across program activities.
  • • Plan and coordinate immunization and other preventive service campaigns with clinical partners.
  • • Develop and deliver guidance for clients and community groups on diagnostic screenings; ensure clear referral pathways.
  • • Design health education initiatives on nutrition, physical activity, and chronic disease prevention.
