Description
Identify and notify people exposed to infectious diseases, conduct interviews to elicit contacts, and provide guidance on testing, isolation, and prevention. Act as a liaison by connecting clients to healthcare, testing, vaccination, and social supports to enable safe isolation or treatment. Maintain confidential records and use public health systems to track cases, contacts, and outcomes. Collaborate with partners to improve service access and response. May assist with outbreak investigations, community outreach, and data collection to inform public health actions.
  • • Interview confirmed or probable cases to obtain symptom history, exposure details, and contact information.
  • • Notify identified contacts of potential exposure while maintaining confidentiality.
  • • Provide cases and contacts with guidance on testing, isolation, or other public health recommendations.
  • • Assess clients for symptoms, risk factors, and support needs using standardized scripts.
  • • Conduct daily or periodic follow-ups to monitor symptoms and adherence to guidance.
  • • Refer or schedule clients for diagnostic testing and vaccination, as appropriate.
  • • Educate clients on infection prevention practices, such as masking, hand hygiene, and ventilation.
  • • Explain when and how to seek medical care or emergency services for worsening symptoms.
  • • Identify and prioritize outreach to high-risk populations or settings.
  • • Document all interviews, outreach attempts, and outcomes in approved surveillance or case management systems.
  • • Ensure informed consent and protect client privacy per HIPAA and program policies.
  • • Provide culturally and linguistically appropriate communication; arrange interpretation as needed.
  • • Deliver written or digital guidance materials in clients’ preferred language.
  • • Coordinate with healthcare providers, laboratories, schools, or employers to verify information and facilitate services, following privacy guidelines.
  • • Develop individualized action plans for testing, isolation, return-to-work or school, and follow-up.
  • • Connect clients to social supports, such as food delivery, paid leave, housing, or transportation assistance, to enable isolation.
  • • Escalate complex, high-risk, or non-responding cases to supervisors or case investigators.
  • • Participate in outbreak investigations in households, workplaces, or congregate settings as directed.
  • • Track and report key metrics, trends, and barriers to inform program improvements.
  • • Provide feedback to public health leadership on improving service accessibility and acceptability.
  • • Use approved tools and scripts to locate hard-to-reach contacts while respecting privacy.
  • • Adhere to productivity, quality, and timeliness standards for outreach and documentation.
  • • Coordinate appointment reminders and follow-up messages via phone, text, email, or mail.
  • • Report suspected abuse, neglect, or threats of harm encountered during interactions, per legal requirements.
  • • Maintain up-to-date knowledge of current guidance and adjust messaging accordingly.
  • • Participate in team meetings, trainings, and quality assurance reviews.
