Description
Provide skilled physical therapy in patients’ homes to restore mobility, reduce pain, and improve safety and independence after illness, injury, or surgery. Assess the home environment, develop and adjust individualized plans of care, educate patients and caregivers, coordinate with the home health team, and document per payer and regulatory standards.
- • Perform comprehensive in-home PT evaluations, including home safety and fall risk assessments.
- • Review physician orders, medical history, medications, and prior records to inform the plan of care.
- • Identify PT diagnoses, functional deficits, and measurable goals relevant to the home setting.
- • Develop and implement individualized home exercise and functional mobility programs.
- • Provide therapeutic exercise, manual therapy, gait and balance training, and neuromuscular re-education.
- • Train patients in safe transfers, bed mobility, stair negotiation, and use of assistive devices.
- • Educate patients and caregivers on home programs, energy conservation, and pain management strategies.
- • Fit, adjust, and instruct in DME and orthotics; coordinate with DME vendors or orthotists as needed.
- • Recommend home modifications and environmental adaptations to improve safety and independence.
- • Monitor vitals and patient response to activity; modify interventions to optimize outcomes.
- • Reassess progress regularly and update goals, visit frequency, and interventions.
- • Obtain and document informed consent and discuss risks, benefits, and alternatives.
- • Document evaluations, visit notes, and progress reports in the EMR per agency and payer requirements.
- • Complete OASIS and other regulatory assessments when applicable.
- • Verify and document homebound status when required by payer policy.
- • Coordinate care and case conference with physicians, nurses, OT, SLP, MSW, and HHAs.
- • Delegate and supervise PTAs and instruct HHAs within scope and regulations.
- • Teach fall prevention, body mechanics, and injury prevention tailored to the home.
- • Apply safe, portable physical agents (e.g., heat/ice, TENS) when indicated.
- • Identify changes in condition and communicate promptly; refer or escalate to appropriate clinicians.
- • Plan discharge, establish a sustainable home program, and arrange follow-up or community referrals.
- • Adhere to infection control, patient identification, and safety protocols in varied home environments.
- • Manage caseload, scheduling, and travel efficiently to meet visit and documentation timelines.
- • Participate in quality improvement, staff education, and evidence-based practice initiatives.
