Description
Provide basic personal care and daily living support to clients in homes, community settings, or care facilities under the supervision of licensed professionals or a care coordinator. Duties include monitoring health and safety, assisting with bathing, dressing, grooming, toileting, mobility, meal preparation, light housekeeping, and supporting transport and companionship. May provide medication reminders and perform simple, noninvasive health-related tasks as outlined in the care plan.
  • • Assist clients with self-administration of medications and noninvasive treatments per care plan and supervision.
  • • Respond promptly to client requests for assistance in person or via call systems.
  • • Apply compression stockings, slings, or simple dressings as directed.
  • • Assist healthcare staff with basic care and simple equipment setup as instructed.
  • • Change bed linens and make beds.
  • • Clean and sanitize living, bathroom, and care areas to maintain infection control.
  • • Assist with collection of noninvasive specimens, such as urine or stool, as directed.
  • • Provide companionship and communicate to assess needs and offer social and emotional support.
  • • Document and report observations, complaints, or changes in condition to supervisors.
  • • Feed clients or assist with eating and drinking.
  • • Review and follow care plans from nurses, therapists, or caregivers.
  • • Measure and record intake and output, reporting significant changes.
  • • Observe clients for symptoms needing attention, such as bruises, skin breakdown, or blood in urine.
  • • Assist clients to position safely for comfort, therapies, or examinations.
  • • Prepare and serve meals or snacks according to care plan.
  • • Provide physical support with activities of daily living, including bathing, dressing, toileting, standing, walking, and exercises.
  • • Record height and weight as scheduled.
  • • Take and record vital signs, such as temperature, blood pressure, pulse, and respirations, as directed.
  • • Remind clients to take medications or nutritional supplements.
  • • Restock personal hygiene items and household supplies.
  • • Review dietary restrictions, allergies, and preferences to ensure appropriate meals.
  • • Set up and maintain simple home-care equipment, such as oxygen or nebulizers, as instructed.
  • • Maintain and organize basic care and medical supplies.
  • • Supply, collect, and empty bedpans or urinals.
  • • Escort or transport clients to appointments or within facilities using wheelchairs or stretchers.
  • • Turn and reposition clients who are bedridden to prevent pressure injuries.
  • • Undress, wash, and dress clients who cannot do so independently.
  • • Provide grooming, shaving, and hygiene care; prepare clients for treatments or exams.
  • • Assist with range-of-motion and mobility exercises as prescribed.
  • • Reinforce provider instructions with clients or families and escalate questions to clinicians.
  • • Lift, transfer, and move clients safely using proper body mechanics and assistive devices.
  • • Provide basic, nonclinical information to clients, families, or visitors and route inquiries appropriately.
  • • Pick up or deliver specimens, medications, or supplies per policy and with proper documentation.
