Description
Transcribe and edit medical reports dictated by physicians and other clinicians using digital dictation systems, converting abbreviations and jargon into clear, complete text. Ensure accuracy, consistency, and proper medical terminology; format and return reports electronically or in print for provider review, signature, and record inclusion.
  • • Transcribe dictation for histories, physicals, emergency room notes, operative reports, consultations, imaging and lab summaries, and discharge summaries.
  • • Edit drafts for spelling, grammar, clarity, consistency, and correct medical terminology.
  • • Expand abbreviations and translate medical jargon into standard, fully understandable language.
  • • Verify terms, medications, and anatomy using approved medical references.
  • • Distinguish homonyms and resolve inconsistencies in dictated content.
  • • Flag and clarify discrepancies or missing information with providers.
  • • Format documents to facility standards, templates, and style guidelines.
  • • Submit completed reports for provider review, signature, and inclusion in the EHR.
  • • Perform data entry and retrieval in EHR and transcription platforms.
  • • Produce medical correspondence, summaries, statistics, and administrative documents as needed.
  • • Maintain and organize digital files and databases of reports and patient histories.
  • • Protect patient information and comply with HIPAA and confidentiality requirements.
  • • Track work queues and respond to status inquiries within privacy guidelines.
  • • Operate transcription equipment, foot pedals, headsets, and transcription software.
  • • Edit and finalize speech recognition drafts to ensure accuracy and completeness.
