Description
Provide person-centered support to individuals with intellectual and developmental disabilities in home and community settings. Promote health, safety, and independence by assisting with daily living, skill-building, and community participation under the guidance of supervisors or clinicians. Monitor well-being, implement individualized support plans, and document services. May assist with medication administration and light household tasks per agency policy and state regulations.
- • Follow Individual Support Plans and document progress toward goals.
- • Assist with personal care, dressing, grooming, and toileting.
- • Support safe meal planning, preparation, and feeding per dietary needs.
- • Coach skills for independent living, money management, and household routines.
- • Facilitate community inclusion, recreation, and transportation to activities or appointments.
- • Monitor and record health status, behaviors, and incidents; report concerns to supervisor or nurse.
- • Prompt or administer medications per training and policy; ensure adherence and accurate logs.
- • Assist with mobility, transfers, and use of adaptive equipment.
- • Implement behavior support strategies and use de-escalation techniques.
- • Teach and reinforce communication and social skills.
- • Maintain a clean, safe home environment, including laundry and light housekeeping.
- • Coordinate with families, guardians, and interdisciplinary team members.
- • Support employment or day program participation, including practicing job skills.
- • Encourage choice, self-advocacy, and rights, respecting privacy and dignity.
- • Accompany individuals on community trips, providing assistance and companionship.
